Junior soccer registration form

Use this form to register the children participating iin Junior Soccer. On submission you will receive a copy of your request and your enrolment will be processed by the City of Stirling.

Terms and conditions

  1. No refunds, credits or makeup classes will be available for missing or withdrawing from a class
  2. Where a withdrawal from a class is requested, a credit or refund maybe issued upon presentation of a medical certificate
  3. Refunds will incur an administration fee
  4. I acknowledge that participation in Stirling Leisure Centres programs by myself, family members and/or my visitors is at our own risk
  5. I also understand that no liability of personal injury, loss or damage to personal effects is accepted by Stirling Leisure Centres, the City of Stirling or its employees whilst attending this program.
