Auslan logoThe City of Stirling has developed a program called Connect2 (formerly known as Community Connections) that assists in connecting senior residents with their local community through transport and small social group outings.

Who can access this service?

City of Stirling residents who are Home and Community Care (HACC) eligible. 

How to arrange an assessment

You need to contact the Central Regional Assessment Service on 1300 785 415 and they will conduct an initial screening over the phone. If you are eligible they will then refer your request to the HACC Metropolitan Regional Assessment Service for a face to face assessment if appropriate. An assessor will contact you to arrange a suitable time for an assessment.

What types of activities are available?

Residents can socialise in small informal groups of around 6-8 people supported by staff and volunteers.  The group goes out in the bus for short, local outings and transport is provided to and from their homes.

What days does the program run?

The small group outings take place Mondays, Wednesdays  and Thursdays each week from 8.30am - 12noon.
